In ACHIEVE-4, participants taking Foundayo experienced a 16% lower risk of MACE-4 and a 23% lower risk of MACE-3, meeting the primary objective for non-inferiority compared to insulin glargine

In pre-planned analyses, Foundayo showed a 53% lower risk of cardiovascular death and a 57% lower risk of all-cause death vs. insulin glargine, suggesting the potential for more comprehensive health benefits

INDIANAPOLIS, Sept. 30, 2026 /PRNewswire/ -- Eli Lilly and Company (NYSE: LLY), the maker of Zepbound (tirzepatide), today announced detailed results from the Phase 3 ACHIEVE-4 trial evaluating the efficacy and safety of Foundayo (orforglipron) compared to titrated insulin glargine in adults with type 2 diabetes and obesity or overweight at increased cardiovascular risk. In the trial, which is the largest and longest study of this medicine in type 2 diabetes to date, Foundayo demonstrated a non-inferior risk of major adverse cardiovascular events (MACE) and improved A1C, weight and other key measures of cardiometabolic health compared with insulin glargine. Results from the trial were presented at the 62nd Annual Meeting of the European Association for the Study of Diabetes (EASD) in Milan, Italy, and simultaneously published in The Lancet.

"For people living with type 2 diabetes, protecting the heart is as important as controlling blood sugar," said Klara Klein, M.D., Ph.D., Assistant Professor of Medicine, University of North Carolina School of Medicine. "ACHIEVE-4 reaffirmed the safety of orforglipron and demonstrated an association with a lower risk of major cardiovascular events and death. These results, paired with the simple administration of a once-daily pill, taken without fasting or water restrictions, could make orforglipron a compelling potential option across the globe."

In the trial, the risk of cardiovascular death, heart attack, stroke, or hospitalization for unstable sudden chest pain (MACE-4) was 16% lower for Foundayo vs. insulin glargine (hazard ratio: 0.84; 95.0% CI: 0.59 to 1.20), meeting the prespecified criteria for demonstrating non-inferiority (upper limit of 95.0% CI of the hazard ratio 1.8).1 The risk of cardiovascular death, heart attack, or stroke (MACE-3) was 23% lower for Foundayo vs. insulin glargine (hazard ratio: 0.77; 95.0% CI: 0.52 to 1.13). In pre-planned analyses, the risk of cardiovascular death was 53% lower (hazard ratio: 0.47; 95.0% CI: 0.25 to 0.90) and the risk of all-cause death was 57% lower (hazard ratio: 0.43; 95.0% CI: 0.25 to 0.75) for Foundayo vs. insulin glargine.2

In addition to demonstrating cardiovascular safety, Foundayo was associated with meaningful improvements across multiple cardiovascular risk factors. At week 52, participants treated with Foundayo experienced reductions in waist circumference by 7.0 cm, systolic blood pressure by 4.4 mmHg, triglycerides by 18.9%, non-HDL cholesterol by 6.6% and hsCRP, a marker of inflammation, by 38.9%.2 Foundayo was also associated with a slower decline in kidney function, a key contributor to cardiovascular risk, including a 2.8 mL/min/1.73 m² smaller reduction in eGFR and a 27.1% greater reduction in albuminuria compared to insulin glargine.2

ACHIEVE-4 Detailed Results

"Foundayo has consistently delivered strong efficacy in type 2 diabetes across our Phase 3 ACHIEVE program," said Thomas Seck, M.D., senior vice president of product development, Lilly Cardiometabolic Health. "In ACHIEVE-4, our largest and longest study of this medicine, Foundayo helped more than half of participants reach an A1C of 6.5% or lower, with durable reductions in A1C and body weight and a favorable overall safety profile. This kind of efficacy reinforces our confidence that Foundayo could change how physicians and patients approach the treatment of type 2 diabetes."

The overall safety and tolerability profile of Foundayo in ACHIEVE-4 was generally consistent with previous trials and with other incretins. The most common adverse events for participants taking Foundayo were nausea, vomiting, diarrhea, decreased appetite, and constipation. During the 52-week minimum treatment period, 10.6% of participants taking Foundayo discontinued treatment due to adverse events.

To further evaluate the impact Foundayo has on cardiovascular outcomes in adults with obesity or overweight with or without type 2 diabetes, Lilly is conducting ATTAIN-OUTCOMES, an ongoing Phase 3 study in adults with established cardiovascular or kidney disease.

About Foundayo (orforglipron)

Foundayo™ (orforglipron) is FDA-approved for adults with obesity, or some adults with overweight who also have weight-related medical problems, to reduce excess body weight and maintain weight reduction long term, alongside a reduced-calorie diet and increased physical activity. Foundayo is a once-daily small molecule (non-peptide) oral glucagon-like peptide-1 receptor agonist that can be taken any time of the day with no planning around food or drink.

Orforglipron was discovered by Chugai Pharmaceutical Co., Ltd. and licensed by Lilly in 2018. In addition to chronic weight management, orforglipron is being studied as a potential treatment for type 2 diabetes, obstructive sleep apnea, osteoarthritis knee pain, hypertension, peripheral artery disease and stress urinary incontinence.

About ACHIEVE-4 and ACHIEVE clinical trial program

ACHIEVE-4 (NCT05803421) is a Phase 3, event-driven, randomized, open-label trial evaluating the efficacy and safety of Foundayo compared with insulin glargine U-100 in adults with type 2 diabetes and obesity or overweight (BMI ≥25 kg/m2) with increased risk of cardiovascular events. The trial randomized 2,749 participants across the U.S., Argentina, Austria, Brazil, Czechia, Germany, Greece, India, Italy, Mexico, Puerto Rico, Romania, Slovakia, South Korea, Spain and Turkey to receive either escalating doses of Foundayo or insulin glargine. The primary objective of the study was to demonstrate that Foundayo was non-inferior in the risk of major adverse cardiovascular events (MACE-4) compared to insulin glargine in adults with type 2 diabetes and obesity or overweight with increased risk for cardiovascular events who have taken at least one and no more than three oral anti-diabetic medications for at least 90 days prior to study start. Study participants had an A1C between ≥7.0% and ≤10.5% (≥7.5% and ≤10.5% if background diabetes medication included a sulfonylurea) and a BMI of ≥25 kg/m2 with stable weight (±5%) for at least 90 days prior to study start. Participants randomized to Foundayo initiated treatment with 0.8 mg once daily and increased the dose every four weeks until reaching a maximum tolerated dose (up to 17.2 mg). The trial was conducted using an investigational formulation of Foundayo at dosages equivalent to Foundayo tablets.

The ACHIEVE Phase 3 global clinical development program for Foundayo began in 2023 and enrolled more than 6,000 people with type 2 diabetes across five global registration trials.

Endnotes

- Time to first event analysis using Cox proportional hazard model using all randomized participants exposed to ≥1 dose of study intervention.

- All-cause death, waist circumference, serum lipids, blood pressure, hsCRP, cardiovascular death and kidney function endpoints were not controlled for family-wise type 1 error. Waist circumference, eGFR, albuminuria, serum lipids, blood pressure and hsCRP were based on efficacy estimand.

- The efficacy estimand represents efficacy had all randomized participants remained on study intervention (with possible dose interruptions and/or dose modifications) without initiating additional antihyperglycemic medications (>14 days of use).

QFIN Investor Notice: Shareholder Rights Law Firm Robbins LLP Reminds Investors of the Class Action Lawsuit Against Qfin Holdings, Inc.
Image File
Original HTML

SAN DIEGO, Sept. 30, 2026 /PRNewswire/ -- Shareholder rights law firm Robbins LLP informs investors that a class action was filed on behalf of persons and entities who purchased or otherwise acquired Qfin Holdings, Inc. (NASDAQ: QFIN) securities between March 18, 2026 and August 25, 2026, inclusive (the "Class Period"). Qfin, together with its subsidiaries, operates a purported artificial intelligence-("AI") driven credit technology platform under the "Qifu Jietiao" brand in the People's Republic of China ("China" or the "PRC").

The complaint alleges that Qfin Holdings, Inc. misled investors regarding the impact of the new PRC regulatory guidance on its business prospects.

Investors who suffered significant losses during the Class Period may be eligible to participate in the lawsuit and should contact Robbins LLP before the November 30, 2026, lead plaintiff deadline.

Why Was Qfin Sued?

According to the complaint, during the Class Period, defendants touted Qfin's ability to weather, adapt, and thrive in response to the recent and emerging PRC regulations affecting its business. For example, at all relevant times, defendants characterized Qfin's business as, inter alia, "resilient", "steady", and "stable" in the face of these regulations, while touting the Company's purportedly "proactive" strategies and policies designed to improve, and which were purportedly already improving, Qfin's risk performance and other operational metrics.

Specifically, defendants failed to disclose that:

(i) defendants had overstated the resiliency and stability of Qfin's business and financial results despite regulatory changes;

(ii) defendants likewise downplayed the true scope and severity of the negative impact that regulatory headwinds were likely to have, and were already having, on the Company's business and financial results; and

(iii) as a result, Defendants' public statements were materially false and misleading at all relevant times.

Why Did Qfin Stock Drop?

Plaintiff alleges that on August 25, 2026, Qfin announced its second quarter ("Q2") and interim 2026 unaudited financial results. The Company reported, inter alia, that total net revenue fell 31.6% year-overyear ("Y/Y") from approximately RMB 5.22 billion to approximately RMB 3.57 billion, significantly missing consensus estimates. In addition, net income plummeted 76.8% Y/Y from approximately RMB 1.73 billion to RMB 401.4 million, heavily impacted by an unexpected RMB 500 million tax expense "caused by a change in tax treatment of certain entities based on the updated interpretation of related tax regulations by the tax authorities." Management also issued disappointing forward financial guidance, projecting a 67% to 73% Y/Y decrease in third quarter ("Q3") non-GAAP2 net income due to rising funding costs and systemic liquidity shocks in the Chinese consumer credit market.

In discussing these disappointing results, Qfin's CEO cited, inter alia, "a challenging market environment marked by continued industry contraction, tighter regulatory oversight, and a sudden industry-wide liquidity shock in late June", warning that, "[l]ooking ahead, we expect industry adjustments to continue, with funding conditions and risk management likely to remain under pressure." On this news, Qfin's American depositary share ("ADS") price fell $2.18 per ADS, or 18.91%, to close at $9.35 per ADS on August 26, 2026.

Who May Be Eligible to Participate in the Qfin Class Action?

The lawsuit seeks to represent investors who purchased or otherwise acquired Qfin Holdings, Inc. securities between March 18, 2026 and August 25, 2026. Investors who suffered losses during that period may have legal rights under the federal securities laws.

SMPL Investor Alert - The Simply Good Foods Company Stockholders with Large Losses Should Contact Robbins LLP for Information About the Securities Fraud Class Action Lawsuit
Image File
Original HTML

SAN DIEGO, Sept. 30, 2026 /PRNewswire/ -- Shareholder rights law firm Robbins LLP reminds investors that a class action was filed on behalf of all purchasers of The Simply Good Foods Company (NASDAQ: SMPL) common stock between October 24, 2024 and April 8, 2026 (the "Class Period"). Good Foods sells consumer packaged foods and snacking products under its various brands.

The complaint alleges that Good Foods misled investors regarding the successful integration of OWYN.

Investors who suffered significant losses during the Class Period may be eligible to participate in the lawsuit and should contact Robbins LLP for information about becoming lead plaintiff before the October 13, 2026 deadline.

Listen to our podcast.

Why Was Good Foods Sued?

Plaintiff alleges that on April 29, 2024, Good Foods announced it agreed to acquire OWYN for $280 million in an all-cash transaction (the "Acquisition"). At the time, Good Foods stated that the Acquisition offered several "compelling strategic and financial benefits," including the diversification of the Company's portfolio by increasing its "presence within the RTD shake segment."

The Acquisition was completed on June 13, 2024, and defendants claimed the integration was "progressing as planned." Then, in October 2025, Good Foods' CEO represented that the OWYN integration had been "largely completed" and "gone well."

The complaint alleges that the integration of OWYN has been a complete failure. Specifically, the complaint alleges that defendants failed to disclose:

(a) that Good Foods had lost key managerial personnel following the Acquisition necessary for the successful integration of the acquired OWYN assets, impairing the Company's ability to achieve the Acquisition's purported strategic initiatives and financial and operational targets;

(b) that Good Foods had materially increased its general and administrative spending to compensate for the loss of key managerial personnel, leading to an inefficient and bloated organizational structure and the lack of clear and cohesive strategic priorities for its OWYN segment;

(c) that the addition of a new pea protein supplier for OWYN formulations prior to the Acquisition had created significant product quality issues which had negatively impacted

the taste, texture, and shelf-life of OWYN products, leading to negative product reviews, depressed consumer sales, and the loss of important distributor relationships;

(d) that, to boost sales in the short-term, Good Foods had offered discounts and engaged in other promotional activities for OWYN products above its historical practices, eroding the Company's margins but failing to achieve the desired sales turnaround;

(e) that, to stem the margin erosion being suffered in its OWYN segment, Good Foods had cut brand support and marketing for OWYN, further depressing product sales; and

(f) as a result of (a)-(e) above, the Acquisition had largely failed to achieve its key strategic goals, the integration of OWYN had run into severe operational and execution problems, and the business and operational results for the Company's OWYN segment had been materially negatively impacted, undermining the Acquisition's economic rationale.

Why Did SMPL Stock Drop?

Plaintiff contends that on April 9, 2026, Good Foods announced disappointing second quarter of 2026 earnings results. First, OWYN's quarterly sales had contracted by nearly 17% year-over-year. Second, Good Foods revealed a $187 million impairment charge against its OWYN brand intangible assets. Finally, the Company slashed its 2026 net sales outlook to a range of negative 7% to negative 10%. On this news, the price of Good Foods common stock declined from $14.41 per share on April 8, 2026, to $10.44 per share on April 10, 2026, a decline of more than 27% over a two-day trading period.

On July 9, 2026, Good Foods reported the Company's financial results for its third fiscal quarter ending May 30, 2026 and revealed an additional $13 million impairment of its OWYN assets, bringing cumulative impairments for the brand to $200 million – 70% of the purchase price less than two years after the Acquisition closed. In the related earnings call, defendant Scalzo acknowledged the severity of the Company's problems following the Acquisition, stating, "[i]mportantly, we remain in the early stages of our turnaround and have significant work ahead."

The price of Good Foods shares has declined more than 70% from Class Period highs of more than $40 per share to lows of less than $11 per share by Class Period end.

Who May Be Eligible to Participate in the Good Foods Class Action?

The lawsuit seeks to represent investors who purchased or otherwise acquired Good Foods securities between October 24, 2024 and April 8, 2026. Investors who suffered losses during that period may have legal rights under the federal securities laws.

What Is a Lead Plaintiff?

The lead plaintiff is a court-appointed investor who represents the interests of all class members throughout the litigation. Shareholders who wish to seek appointment as lead plaintiff must do so no later than October 13, 2026.

Serving as lead plaintiff is not required to share in any potential recovery. Investors who do not seek appointment may remain absent class members if the case proceeds and later resolves successfully.

Ethiopian Airlines Orders Boeing 777 and 777-8 Freighters to Expand Cargo Fleet

- Firm order includes eight 777-8 Freighters and two 777 Freighters

- First African carrier to purchase the world's newest widebody freighter, the 777-8 Freighter

- Ethiopian Airlines doubles its 777X family order book

ADDIS ABABA, Ethiopia, Sept. 30, 2026 /PRNewswire/ -- Boeing (NYSE: BA) and Ethiopian Airlines today announced the African carrier has ordered eight 777-8 Freighters and two 777 Freighters. The airline will pair the proven performance of the 777 Freighter with the next generation 777X capabilities of the new 777-8 Freighter to expand its Boeing freighter fleet.

The order is Ethiopian Airlines' second for the 777X airplane family, building on its purchase of eight 777-9 passenger jets that established the carrier as Africa's first 777X customer. The 777-8 Freighter will be the world's largest and most capable twin-engine freighter, supporting Ethiopian Airlines' expanding global network and advancing its international freight operations.

"This agreement marks another significant milestone in strengthening Ethiopian Airlines' cargo capabilities while supporting the continued expansion of our global network," said Mesfin Tasew, Group CEO of Ethiopian Airlines. "The addition of the Boeing 777-8F Freighters and 777F Freighters will enhance our ability to serve customers around the world with greater payload capacity, operational flexibility, efficiency, and sustainability. As demand for cargo services continues to grow, these aircraft will play a vital role in facilitating global trade, strengthening supply chain connectivity, and further reinforcing Ethiopia's position as a leading cargo gateway between Africa and international markets. It also marks our long-term partnership with Boeing."

With a maximum structural payload of 118 tonnes, the 777-8F offers the highest payload and range capability to open new markets while reducing fuel use and delivering the lowest operating cost per tonne of any freighter. The 777F set the standard for twin-engine freighter efficiency and productivity, and Ethiopian Airlines' 777F fleet ranks among the most highly utilized commercial airplanes in service.

"Ethiopian Airlines' order for the industry-leading 777 Freighter and new 777-8 Freighter highlights both the strength of our partnership and growing demand for air cargo worldwide," said Brad McMullen, Boeing senior vice president of Commercial Sales and Marketing. "We appreciate Ethiopian Airlines' continued confidence in Boeing and the 777 and 777X family of airplanes as it expands its cargo capabilities and global network. The airline continues to make history as the first in Africa to order the new 777-8 Freighter."

The 777-8F will complement Ethiopian Airlines' freighter fleet, retaining the key features of the 777 Freighter that support efficient operations, high utilization and reliability, including common cross-sections and walkways, and easy carriage of large outsize cargo.

Ethiopian Airlines currently connects more than 70 cargo markets across Africa, Asia, Europe, the Middle East and North America with a fleet of:

- 12 777 Freighters

- Two 767 Freighters

- And four 737-800SF airplanes

The new order will enable the airline to build on this network by providing greater flexibility to meet rising global demand for air cargo.

A leading global aerospace company and top U.S. exporter, Boeing develops, manufactures and services commercial airplanes, defense products and space systems for customers in more than 150 countries. The scope covers local, regional, national, and international media outreach, athlete and sponsor storytelling, social content, and reputation management across the full quadrennial, including the 2029 World Championships in Park City, Utah, and the 2030 Winter Games in France.

"Bobsled and skeleton athletes train for four years for a run that lasts under a minute," said Joseph Alexander, CEO of The Uptown Agency. "Our job is to make sure the country knows their names long before they reach the start line. We are honored that USABS has trusted us with that, and we are building a program that keeps these athletes and this organization visible between Games, not just during them."

The partnership is already underway as USABS begins a new sliding season. The 2026 USA Push Championships take place Oct. 5–10 in Lake Placid, New York. The first competition of the 2026–27 season, Push Championships give athletes the opportunity to demonstrate their speed and power after a long offseason, with hundredths of a second often separating the field.

Although masked, the creative features team member and hometown hero, Sam Allred of Plano. The Uptown Agency is also helping USABS with social efforts featuring athletes like Olympic bronze medalist Jasmine Jones and a partnership with Polar Seltzer.

The Uptown Agency brings a sports communications track record that spans the NBA, Major League Baseball, Major League Soccer, and NASCAR. The agency helped USABS launch their 50 States Strong campaign at the beginning of the 2026 Winter Olympics, which included a video voiced by Josh Lucas, and a track created by SHINDIG Music.

HelloNation Article Featuring Tucson Tile Roofing Expert Drew Nelson Explains Why Hairline Cracks on Tile Roofs Lead to Larger Repairs
Original HTML

The article outlines how small tile roof cracks can allow moisture intrusion and weaken long-term roof performance in Arizona's climate.

CATALINA FOOTHILLS, Ariz., Sept. 30, 2026 /PRNewswire/ -- What causes hairline cracks to turn into major problems on a tile roof? That's the key question answered in a HelloNation article featuring insights from Drew Nelson, a Catalina Foothills roofing professional and owner of Apex Roof Pros LLC. The piece explains how even the smallest signs of damage—like hairline cracks—can lead to significant repairs for homeowners across Tucson, especially on roofs built with clay or concrete tiles.

The article explains that hairline cracks on a tile roof are not just cosmetic issues. According to the HelloNation piece, these small openings can allow water to move beneath the tile surface. When moisture reaches the underlayment—the layer responsible for waterproofing—the roof's overall protection can slowly degrade. In the dry, high-heat climate of Tucson, Nelson shares that heat cycles can make this worse. The constant expansion and contraction of tiles during day and night causes cracks to widen, which puts long-term pressure on the roof system.

As a Tucson tile roofing expert, Nelson has seen how untreated cracks weaken the structure. The HelloNation article explains how cracked tiles shift weight unevenly, creating stress points. When a monsoon or windstorm hits, the added pressure can cause a tile to slip or break, exposing the underlayment. Once exposed, the system no longer functions as intended, and repairs become more complex.

The article emphasizes that routine roof inspections are key. In Catalina Foothills, roofing professionals like Nelson use these checks to spot problems early. He points out that many homeowners wrongly assume tile roof cracks aren't urgent. But left alone, hairline cracks can invite moisture, dust, and debris under the tile, eventually harming the underlayment. That kind of hidden damage costs more to fix than simply replacing a tile.

Another factor Nelson highlights in the article is the quality of the original tile installation. Properly installed clay and concrete tiles are designed to last for decades, but the HelloNation feature notes that shifting tiles and installation mistakes can accelerate wear. In Catalina Foothills roofing, where high temperatures and constant heavy UV exposure are the norm, even small imperfections can grow over time.

One detail the article points out is that Drew Nelson's work aligns with standards set by the Tile Roofing Alliance. This industry group promotes best practices for fastening tiles and managing thermal movement. According to HelloNation, following those standards helps roofing experts like Nelson ensure lasting results and consistent performance on every repair job.

In the Tucson tile roofing market, local weather conditions make early action even more important. Catalina Foothills homeowners often wait too long to deal with minor roof damage. The HelloNation article warns that this can lead to broader issues, including energy loss due to gaps in insulation. When a tile roof is not sealed tightly, it can affect how well the home withstands desert heat.

By promptly addressing tile roof cracks, homeowners can avoid costly replacements and extend their roof's lifespan. With proper care from certified professionals, clay and concrete tiles can remain durable for many years. But as Nelson explains, staying ahead of hairline cracks is essential in the Arizona climate.

Why Hairline Cracks Matter on Tile Roofs in Tucson features insights from Drew Nelson, Roofing Expert of Catalina Foothills, AZ, in HelloNation.

A Hustle Worth Having is supported by a FEMA SAFER grant. More than 60% of all fire and EMS departments in the U.S. are staffed by volunteers. In some states, this percentage is even higher, over 80%. Maintaining minimum staffing is increasingly challenging for departments across the country.

As the IAFC continues to champion the vital role of volunteer firefighters in safeguarding communities, the launch of this new national campaign underscores their commitment to innovation and collaboration with departments. By equipping them with the necessary tools and resources, this initiative aims to foster a resilient and dedicated volunteer firefighting force, ensuring the safety and well-being of communities for years to come.

Late-Breaking EADV 2026 Data Support Use of DecisionDx®-Melanoma in SLNB Decision-Making and Risk-Aligned Post Surgical Management
Image File
Original HTML

Data show patients with low-risk DecisionDx-Melanoma i31-SLNB results had sentinel lymph node (SLN) positivity below 5% when biopsied, and eligible patients who avoided SLNB had five-year recurrence-free survival (RFS) of 99.6%

Separate ePoster shows DecisionDx-Melanoma identifies early-stage, transected melanomas with recurrence risk similar to that seen in stage IIB disease, supporting closer surveillance when staging is uncertain

FRIENDSWOOD, Texas, Sept. 30, 2026 /PRNewswire/ -- Castle Biosciences, Inc. (Nasdaq: CSTL), a company improving health through innovative tests that guide patient care, will present new data on DecisionDx-Melanoma at the European Academy of Dermatology and Venereology (EADV) Congress 2026, taking place Sept. 30–Oct. 3 in Vienna, Austria.

A late-breaking oral presentation will highlight the largest prospective, multi-center study of gene expression profiling (GEP) in cutaneous melanoma to date, involving 2,799 patients across 29 U.S. centers. The findings support using DecisionDx-Melanoma to identify patients who may safely forgo SLNB and patients with early-stage disease whose higher recurrence risk may warrant closer surveillance.

"Staging is an essential starting point, but understanding a patient's tumor biology can help us make more informed recommendations about sentinel lymph node biopsy and follow-up care," said Joseph Gadzia, M.D., study author and board-certified dermatologist and Mohs micrographic surgeon at Kansas Medical Center in Topeka, Kansas. "Seeing excellent outcomes among patients with low-risk i31-SLNB results who did not undergo SLNB in this large, prospective study should give physicians greater confidence in using DecisionDx-Melanoma results to help guide these important decisions with their patients."

Late-Breaking Oral Presentation

- Abstract LB-339: The integrated 31-gene expression profile (i31-SLNB) identifies patients with a low risk of sentinel lymph node positivity and 31-GEP identifies patients with high 5-year recurrence risk: The largest prospective, multicenter study of GEP for cutaneous melanoma

- Session: D2T01.3 - Late Breaking News

- Room: Hall A

- Date and Time: Thursday, Oct. 1; 14:45 - 15:00 CEST

- Key findings: This multi-center, prospective study enrolled 2,799 patients across 29 U.S. centers to evaluate DecisionDx-Melanoma's ability to predict sentinel lymph node (SLN) positivity and stratify recurrence risk.

- Among 676 patients who underwent SLNB and received an i31-SLNB result, those classified as low risk (5% predicted risk) had an observed SLN positivity of 4.4%, consistent with the test's predicted risk of less than 5%. By comparison, patients classified as high risk (>10% predicted risk) had an observed SLN positivity rate of 22.8%.

- Among 565 SLNB-eligible patients with low-risk i31-SLNB results who did not undergo the SLNB procedure, five-year RFS was 99.6%.

Together, these findings support using the test's i31-SLNB result to identify low-risk patients who may safely forgo SLNB and patients whose higher-risk result supports consideration of the procedure.

DecisionDx-Melanoma's 31-GEP Class result also significantly stratified five-year recurrence risk and provided prognostic information beyond traditional staging. Patients with early-stage disease (stage I–IIA) and high-risk Class 2B results had RFS similar to patients with stage IIB disease, supporting consideration of closer surveillance and additional treatment for patients whose higher risk may not be fully captured by staging alone.

A second abstract on DecisionDx-Melanoma will also be shared at EADV via an ePoster.

- Title: The 31-gene expression profile test for cutaneous melanoma accurately identifies patients with transected tumors at high and low 5-year risk of recurrence

- Key findings: When a biopsy cuts through the base of a melanoma, known as transection, the sample may not capture the tumor's full thickness. Because thickness helps determine melanoma stage, this can leave uncertainty about a patient's risk and how closely they should be monitored.

In a multi-center analysis of 687 patients with transected melanomas, DecisionDx-Melanoma significantly stratified patients at high and low risk of recurrence. Five-year RFS was 93.2% for patients with low-risk (Class 1A) results versus 64.1% for those with high-risk (Class 2B) results (p0.001). Among patients with early-stage disease (stage I–IIA), those with Class 2B results had five-year RFS of 73.7%, similar to those with stage IIB melanoma and worse than those with stage IIIA.

These findings support using DecisionDx-Melanoma to help tailor care when initial staging is uncertain. Low-risk (Class 1A) results, which were most common in the study, may support less intensive follow-up. High-risk (Class 2B) results in patients with early-stage disease may warrant closer surveillance and consideration of imaging that would not typically be recommended based on their stage alone.

Castle will also present an encore of previously reported validation data for its AdvanceAD-Tx™ test via an ePoster titled, "The 487-gene expression profile test guides systemic therapy selection to improve outcomes for patients with atopic dermatitis: Results from a prospective, multi-center trial." These data have now been published in the Journal of the American Academy of Dermatology.

About DecisionDx-Melanoma

DecisionDx-Melanoma is a gene expression profile (GEP) test designed to analyze tumor biology to deliver a personalized risk assessment for patients with stage I–III cutaneous melanoma, enhancing risk stratification beyond American Joint Committee on Cancer (AJCC) staging alone. By combining molecular insights with select clinicopathologic features, the test provides two distinct outputs: a personalized risk of sentinel lymph node (SLN) positivity and a personalized risk of recurrence and/or metastasis. This clinically actionable information is designed to help guide risk-aligned patient management decisions, including SLN biopsy consideration, follow-up intensity, imaging and referrals.

DecisionDx-Melanoma is supported by 58 peer-reviewed publications, including prospective studies and meta-analyses, and was developed in collaboration with more than 100 leading U.S. institutions. The test has been clinically validated in more than 10,000 patient samples, ordered more than 240,000 times since launch, and has been shown to be associated with improved patient survival. Learn more at www.CastleBiosciences.com.

About AdvanceAD-Tx

AdvanceAD-Tx is a non-invasive gene expression profile (GEP) test designed to guide systemic treatment decisions for patients aged 12 years and older with moderate-to-severe atopic dermatitis (AD). Using RNA expression data from lesional skin scraping samples—no biopsy required—the test evaluates 487 genes across 12 inflammatory and cutaneous biology pathways to reveal the underlying immune biology driving an individual patient's disease. The test classifies patients into one of two molecular profiles: a Janus kinase (JAK) Inhibitor Responder Profile or a T helper type 2 (Th2) Molecular Profile.

The prospective clinical validation study showed that patients with a JAK Inhibitor Responder Profile given JAK inhibitor therapy experienced significantly greater clinical benefit — including improved and faster skin clearance (EASI-90), greater likelihood of achieving no itch and remaining flare-free, and better quality of life by three months — compared to those treated with a Th2-targeted therapy. AdvanceAD-Tx provides clinicians with objective, molecular-based insights to help personalize systemic treatment decisions and improve care for patients.
